If you're planning on getting a flu shot, you can take advantage of a $5 rebate back from Lysol. All you'll need to do is fill out this form, and mail it in with a dated receipt showing payment for your flu shot. This is only valid for flu shot received from 11/7 - 11/30, and must be postmarked 12/30/2010. Offer is valid throughout the US except for Rhode Island and is limited to one rebate per name, address, or household.
